What is the GUIDE model?
The GUIDE (Guiding an Improved Dementia Experience) Model is an innovative program introduced by
the Centers for Medicare & Medicaid Services (CMS). This program is designed to provide comprehensive,
coordinated care for individuals living with dementia. The GUIDE Model’s mission is to improve the quality
of life for dementia patients, reduce the burden on their caregivers and enable individuals to continue
living in their homes and communities for as long as possible.
As a participant in the GUIDE Model, Sabio Health is committed to delivering high-quality care and support
through personalized care plans, caregiver education, and comprehensive care coordination.

How to enroll in GUIDE servies with Sabio Health
If you are a traditional Medicare beneficiary and want to determine your eligibility for GUIDE services, Sabio Health can help. Our providers will conduct a comprehensive assessment to evaluate your needs. If you meet the diagnosis criteria for GUIDE, we will guide you through the enrollment process and provide ongoing support.

No Referral Needed
You do not need a referral to access GUIDE services.

Medicare Coverage
GUIDE services are covered under Medicare at no additional cost for eligible services specified in the model.

By partnering with Sabio Health under the GUIDE Model, providers can:
Enhance care coordination and patient outcomes.
Access caregiver education programs and respite service payments for eligible beneficiaries.
Reduce hospitalizations and emergency room visits, lowering overall long-term care costs.
GUIDE offers a per beneficiary per month care management payment (DCMP), which is adjusted based on health equity and quality performance metrics. Additionally, separate payments are available for respite services for patients with moderate to severe dementia.
