We have purpose-built age-friendly primary care that considers the unique needs of medically underserved older
adults that helps manage complex conditions, improves quality of life, and promotes independence.
Our care teams have specific training in diagnosing and managing the complex health conditions that
frequently affect older adults, such as memory loss, falls, and incontinence. They also have experience
with managing comorbid conditions that are more common in older populations, such as high blood
pressure, diabetes, and heart disease.
